Covid-19 – On-site testing for Confetti college-level students

To help safeguard our Confetti community, we will be offering access to rapid lateral flow coronavirus (COVID-19) tests for college-level students when they return to campus.

The use of rapid lateral flow tests allows us to identify individuals with coronavirus (COVID-19) who do not have symptoms, which make up around a third of all cases. Finding asymptomatic cases, along with other infection prevention and control measures such as social distancing, can help us manage the transmission of the virus.

Testing will be managed and supervised by Confetti’s staff teams, who have been trained in administering rapid lateral flow tests. These tests will be provided free of charge to Confetti students and take approximately 10 minutes.

Testing is not mandatory and we require your consent, but we are encouraging our students to take the tests when made available to them. In some cases, someone who has tested negative may still have the undetected disease and be infectious, so our full campus safety measures remain in place. If you do not provide consent below you cannot take part in the testing programme.

This testing programme does not replace the current testing programme for those with symptoms. Anyone who is showing symptoms of coronavirus (COVID-19) will be required to self-isolate and request a coronavirus test by following the NHS procedure.

We will be using the details entered on this form, along with the result of your test, so we can monitor and report on the impact of the virus at Confetti. Consent is optional, but you will not be able to undertake a test if you haven’t consented.

Confetti Institute of Creative Technologies – COVID-19 Testing Privacy Statement

To enable the Covid-19 testing to be completed at Confetti, we need to process personal data for students taking part, including sharing of personal data where we have a legal obligation. Confetti is the Data Controller for the data required for processing the tests and undertaking any actions which are needed by Confetti to ensure we meet our public health and safeguarding legal obligations.

If you decline a test, we record your decision under the legitimate interest of Confetti in order to have a record of your decisions and to reduce unnecessary contact with you regarding testing.

The processing of special category personal data is processed under the provisions Section 9.2(i) of GDPR, where it is in the public interest on Public Health Grounds. This data is processed under the obligations set out in Public Health legislation (Regulations 3(1) and (4) of the Health Service (Control of Patient Information) Regulations 2002 (COPI)) which allows the sharing of data for COVID related purposes and where it is carried out by someone who owes an equivalent duty of confidentiality to that data.

Data Controllership is then passed to the Department for Health and Social Care (DHSC) for all data that we transfer about you and your test results to them. For more information about what they do with your data please see the Test and Trace Privacy Notice. The establishment remains the Data Controller for the data we retain about you.

We use the following information to help us manage and process the tests:

  • Name
  • Date of birth
  • Cohort
  • Gender
  • Ethnicity
  • Home postcode
  • Email address
  • Mobile number
  • Unique barcode assigned to each individual test and which will become the primary reference number for the tests
  • Test result
  • Parent/guardians contact details (if required)

We will only use information that is collected directly from you specifically for the purpose of the tests, even if you have previously provided us with this information.

The information will only be stored securely in our database and on local password-protected spreadsheets in Confetti whilst it is needed. It will also be entered directly onto DHSC digital services for the NHS Test and Trace purposes. Confetti will not have access to the information on the digital service once it has been entered.

The student will be informed of the result by Confetti and advised how to book a confirmatory test.

We will use this information to enact your own COVID isolation processes without telling anyone who it is that has received the positive test.

The information will be transferred to DHSC, who will share this with the NHS, GPs. PHE and the Local Government will use this information for wider test and trace activities as well as statistical and research purposes.

This information will be kept by Confetti for up to 28 days and by the NHS for 8 years.

We will record a negative result and the information will be transferred to DHSC, NHS, PHE and the Local Government who will use the information for statistical and research purposes.

This information is processed and shared under obligations set out in Public Health legislation under Regulations 3(1) and (4) of the Health Service (Control of Patient Information) Regulations 2002 (COPI) which allows the sharing of data for COVID related purposes.

This information will be kept by Confetti for up to 28 days and by the NHS for 8 years.

We will record that you have declined to participate in a test and this information will not be shared with anyone.

The personal data associated with test results will be shared with

  • DHSC, NHS, PHE – to ensure that they can undertake the necessary Test and Trace activities and to conduct research and compile statistic about Coronavirus
  • Your GP – to maintain your medical records and to offer support and guidance as necessary
  • Local Government to undertake local public health duties and to record and analyse local spreads
  • Access Creative College – as lead provider for the FE contract
  • Nottingham Trent University – as parent company of Confetti Media Group

Under data protection law, you have rights including:

Your right of access – You have the right to ask us for copies of your personal information.

Your right to rectification – You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to erasure – You have the right to ask us to erase your personal information in certain circumstances.

Your right to restriction of processing – You have the right to ask us to restrict the processing of your personal information in certain circumstances.

Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability – You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.

    I have had the opportunity to consider the information provided by Confetti about the testing, ask questions and have had these answered satisfactorily, based on the information presented in the communications dated 02/03/2021 and the attached Privacy Notice.

    I consent to having a nose and throat swab for lateral flow tests. I will self-swab if I am able to otherwise I understand that assistance is available.

    I understand that there may be multiple tests required and this consent covers all tests for the below named person. If, on the day of testing I do not wish to take part, then I understand I will not be made to do so and that consent can be withdrawn at any time ahead of the test.

    I consent that my sample will be tested for the presence of COVID-19.

    I understand that if my results are negative on the lateral flow test I will not be contacted by Confetti except where I am a close contact of a confirmed positive.

    If the lateral flow test indicates the presence of COVID-19, I consent to having a nose and throat swab for confirmatory PCR testing. I will follow the instructions on the PCR Kit to return the test the same day to an NHS Test & Trace laboratory.

    If the lateral flow test indicates the presence of COVID-19, I commit to ensuring that I am removed from Confetti premises as promptly as possible, bearing in mind I may have some anxiety following a positive test result.

    I consent that I will need to self-isolate following a positive lateral flow test result, until the results of the confirmatory PCR have been received.

    I agree that if my test results are confirmed to be positive from this PCR test, I will report this to Confetti and I understand that I will be required to self-isolate following public health advice.

    I consent that if a close contact of my child tests positive but I have tested negative, I will continue to attend school / college but will be tested every day at Confetti for 7 days.
